How To Refer To Us
A written referral is not strictly necessary but is appreciated and assists continuity of care. You may find it easiest to simply ‘copy’ us in any correspondence regarding your patient, or you may choose to print our referral form and fax it to us on (03) 8823 8399. We also suggest printing a copy of this form for your patient. If you would like us to send you a hard copy referral pad, you can order one here, or call our rooms on (03) 8823 8300.
Private Health Insurance and Medicare rebates are available for our services (a GP Enhanced Primary Care plan is required for a Medicare rebate). Any investigations or a copy of correspondence regarding your patient would be appreciated prior to their initial assessment to ensure continuity of care.
