Camberwell

Expert & Personalised Service In Purpose Built Treatment Rooms

Our Camberwell rooms are situated on Burke Road, conveniently just 1km north of the Monash Freeway near the corner of Toorak Road. We have on-site car parking and public transport (tram 72) stops at our front door. Our beautiful light filled rooms were opened in August 2014, when we moved from our home of 20 years in Malvern.

General

Please arrive 5 minutes before your scheduled appointment time and allow approximately 50 minutes for your initial consultation.

Before your visit, our Patient Support Services team will either mail or email you some forms to fill in.  This will often include an extensive pelvic health questionnaire.  We take a whole-person approach to your symptoms, and recognise that many pelvic health conditions have a physical and emotional component to them.  Our Physiotherapists need to know as much about your symptoms and history as possible in order to develop a personalised pelvic health program for you.  Please complete these forms at home at your leisure, leaving any questions that are not relevant or you don’t feel comfortable completing. Please be assured that any information you provide will be kept highly confidential.

Bring these forms with you to your first appointment but don’t worry if you forget them. We have copies at the reception desk which you can fill in when you get here but please arrive about 15 minutes before your appointment in order to give you enough time.

Payment

Payment of account is due at the time of consultation. Cash, cheques, EFTPOS and major credit cards are accepted.

If Workcover, TAC or Comcare are responsible for payment of your account or you are on an Enhanced Primary Care Plan (EPC) or Chronic Disease Management Plan (CDM), please bring these details with you and inform the receptionist upon arrival. Please note that in each of these cases, the compensable rate does not fully cover our professional fee.

DVA patients must bring a doctors referral and DVA card to the first appointment.

Cancellations & Non-attendance Policy

If you are unable to keep this appointment, please call 8823 8300 at least 24 hours prior to the scheduled time. Non-attendance or late cancellation of an appointment will incur a fee.

Rebates

Physiotherapy is not covered under Medicare (unless you have an EPC/CDM plan). Private Health Insurance with “Extras” provides a rebate for physiotherapy treatment costs. (Hicaps facility is available at this location).

Parking

Onsite parking is available at the rear of the building (use the laneway beside our building and ‘Bill’s Fish and Chips’). There is also ample parking on both sides of Burke Rd and in surrounding streets.
